Use this form to report a change in your situation if you are receiving an unemployment benefit (‘WW-uitkering’) and living abroad. You can also use this form to share your foreign bank account number with us.

What you need to complete the form

For changes in work and income, you will need to submit the following documents:

• a payslip of your benefit
• a copy of your rental agreement
• your employment contract
• a payslip of your wage

If you receive a top-up benefit and your partner's income changes, you will need to submit the following documents:

• your partner's citizen service number (‘BSN’)
• your partner's payslip
