If you are receiving an unemployment benefit or an IOW benefit and you want to apply for a top-up benefit, complete the Top-up benefit application form (‘Aanvragen toeslag WW-uitkering of IOW-uitkering’).

If you want to apply for an unemployment benefit and a top-up benefit at the same time, complete the Unemployement benefit application form (‘WW-uitkering aanvragen’) to apply for both.

You will need either a DigiD account or a valid European electronic ID (‘Europees erkend inlogmiddel’) to apply.

If you are applying for, or receiving, a sickness benefit and want to apply for a top-up benefit, complete the Top-up benefit application form (‘Toeslag aanvragen’). Please also use this form if you are not receiving an UWV benefit and are receiving a lower salary because you are ill.

Complete, download and post the application form to us at the address given on the form. 

You might also qualify for a top-up benefit when applying for, or receiving, a WIA, WAO, WAZ or Wajong benefit. To apply for a top-up benefit in such circumstances, complete the Top-up benefit application form when receiving an occupational disability benefit (‘Aanvragen toeslag als u een WIA-, Wajong-, WAO- of Waz-uitkering heeft’). You will need either a DigiD account or a valid European electronic ID (‘Europees erkend inlogmiddel’) to do this.

Please note: You must specify your partner’s income on the application form if you are either married or the equivalent of married.

You will receive a decision about the top-up benefit within 8 weeks of us having received your form.
