    Answering questions about your health, abilities and disabilities

    If you are receiving a WIA benefit, we want to know how you are doing. This is why we would like you to fill in a form with questions about your health, abilities and disabilities. This information will help us determine if we need to re-evaluate your situation.

    How to tell us how you are doing

    You can let us know how you are doing by completing the Questionnaire about your health, abilities and disabilities (‘Vragen over uw gezondheid, mogelijkheden en beperkingen’).

    What will you need to complete the form?

    You will need either a DigiD account or a valid European electronic ID (‘Europees erkend inlogmiddel’) to fill in the form. Please also keep the following information close to hand:

    • the contact details of your physician, therapist or hospital
    • any relevant medical information

    What happens next

    Once you have completed the form you can save or print it for your own records. If the UWV physician (‘verzekeringsarts’) needs any more information, they will contact your health provider. We will let you know if this is the case.
