You can let us know how you are doing by Answering some questions about your health, abilities and disabilities (‘Vragen over uw gezondheid, mogelijkheden en beperkingen’). You will only need to complete this form if you have received a letter from us asking you to do so.

You will need a DigiD account to fill in the form. Please also keep the following information close to hand:

• the contact details of your physician, therapist or hospital
• any relevant medical information

Please note: It takes about 60 minutes to complete the form. It is not possible to save the data you entered until you have completed the form. For security reasons, you will be automatically logged out if you are inactive for more than 15 minutes. All entered data will be lost.

Once you have completed the form you can save or print it for your own records. If the UWV physician (‘verzekeringsarts’) needs any more information, they will contact your health provider. We will let you know if this is the case.
