The data on Mijn UWV and in your overview of your employment history and payslip information comes from employers and benefit organisations that have filed payroll tax returns with the Tax and Customs Administration ('de Belastingdienst').
To access your employment history and payslip information (in Dutch), log in to Mijn UWV using either your DigiD account or a valid European electronic ID (‘Europees erkend inlogmiddel’).
If you spot an error in your employment history or payslip information, you must inform us so we can correct it.
To inform us of the error, complete the Correction application form (‘Correctieverzoek doorgeven wijzigingen in uw arbeidsverleden en loongegevens’). We will forward your request to the Tax and Customs Administration, which will request the data from the employer or benefit organisation.
Please note: If you spot an error in your personal details, such as your name, citizen service number (‘BSN’) or date of birth, you must contact your local council (‘de gemeente’) to correct them.
You may need to include 1 or more of the following supporting documents with your form, depending on your situation:
- Provide your employment history and wage details.
- Provide any corrections to your wage or contract information.
- Provide payslips (‘loonstroken’).
- Provide your employment contract (‘arbeidsovereenkomst’).
- Provide proof of child benefit (‘kinderbijslag’).
Submit this information as soon as possible. We cannot process your correction request without the right supporting documents.
If any documents are missing, we will let you know so that you can upload them. You can use the same form again to upload any additional items.
You will need this overview when applying for a mortgage or loan. Make sure the document is watermarked for mortgage applications as this proves it is a legitimate UWV document. Keep in mind that you must use an online document for your application as this is the only way to ensure the watermark is visible.
Your employment history and payslip information will also be used by us to determine the level and duration of your benefit.
After submitting the form, you will receive confirmation that it has been received.
