    Completing a medical questionnaire when applying for a risk-free scheme extension

    If you qualify for the risk-free scheme for employers (‘no-riskpolis’), we will pay your sickness benefit (‘Ziektewet-uitkering’) to your employer if you become ill. This scheme is valid for 5 years. If you apply for an extension on this scheme, we might also ask you for your medical information.

    Why complete the medical questionnaire?

    We might need your medical information to help us evaluate your application for an extension. You might therefore be asked to complete a medical questionnaire (‘Medische vragenlijst bij aanvraag no-riskpolis verlengen’). To access this questionnaire, log in using either a DigiD account or a valid European electronic ID (‘Europees erkend inlogmiddel’).

    What you will need when completing the form

    Make sure you have the following close to hand when completing the form:

    • information about your illness
    • details about your doctors and specialists 
    • information about your treatment 
    • information about your medication

    Decision

    After you have completed the medical questionnaire, an UWV physician will assess it. You may also be asked to attend a doctor’s appointment. You will usually receive a decision about your application within 8 weeks.
