In cases of serious illness, the UWV physician could tell you that you will be unfit to work for several years. If this happens, you might be able to extend the risk-free scheme for employers. To qualify for an extension, 1 of the following situations must apply to you:

• You already had the illness at the start of your employment.
• You already had the illness and began receiving the WIA-benefit while in employment.

To apply, complete the ‘Application form for extending the risk-free scheme for employers’ (‘aanvraag no-riskpolis verlengen’). You will need to log in with your DigiD account to access this form.

Make sure you keep the following close to hand when completing the form:

• your citizen service number (‘BSN’)
• your employer’s address 
• your contact person’s details

You will receive a decision about your application within 8 weeks of sending us the form. You might also be asked to fill in a medical questionnaire.

Keep in mind that we will not process your application any sooner than 6 months before the statement expires. This is true even if you submit your application at an earlier date.
