If you have a serious illness, your UWV physician might tell you that you will be unfit for work for several years. If this happens, you might be able to extend the risk-free scheme for employers. To qualify for an extension, 1 of the following situations must apply to you:

• You already had the illness at the start of your employment.
• You already had the illness and began receiving the WIA-benefit while in employment.

To apply, complete the ‘Application form for extending the risk-free scheme for employers’ (‘aanvraag no-riskpolis verlengen’). You will need to log in with either a DigiD account or a valid European electronic ID (‘Europees erkend inlogmiddel’) to access this form.

Make sure you have the following close to hand when completing the form:

• your citizen service number (‘BSN’)
• your employer’s address 
• your contact person’s details

You will receive a decision about your application within 8 weeks of sending us the form. You might also be asked to fill in a medical questionnaire.

Keep in mind that we will not process your application any sooner than 6 months before the statement expires. This is true even if you submit your application at an earlier date.
