To claim a disability-related expense, complete the disability-related expenses form (‘Indienen declaratie voor voorzieningen’). You will be able to access this form by logging onto Mijn UWV with your DigiD account.
It is likely that we will ask for receipts or documents that prove you incurred these costs. Please attach your receipts to the form by using the ‘Upload’ button on the ‘Disability-related expenses form’.
If you would rather send your receipts to UWV by post, you can do so. Just don’t forget to fill in the specification sheet as well. Then send the completed specification sheet and your receipts to the address specified on the sheet.
