If you authorise someone, they will be able to do the following:

• receive and send messages about your coverage
• receive and send letters about your coverage
• request and provide information about your coverage
• pay your coverage contributions

To authorise someone, complete the Authorisation form for voluntary income protection coverage (‘Aanvragen gemachtigde voor verzekering van UWV’).

You will need the following information when completing the form:

• your citizen service number
• your bank account number 
• your authorised person’s bank account number 
• your authorised person’s personal information

It is important that you and your authorised person both sign the completed form. Once this has been done, you can upload the form via Mijn UWV. Or send it by post to the address specified on the form.
