Authorising someone to manage your voluntary income protection coverage
If you want someone to manage your voluntary income protection coverage (‘vrijwillige verzekering’) for you, you can authorise them to do so. This could be your employer, accountant or someone else you trust.
What does authorisation mean?
Once you authorise someone to manage your income protection coverage, they will be able to do the following:
- receive and send messages related to your coverage
- receive and send letters about your coverage
- request and share information about your coverage
- pay your premiums
How to authorise someone
To authorise someone, complete the Authorisation form for voluntary income protection coverage (‘Aanvragen gemachtigde voor verzekering van UWV’). You will need either a DigiD account or a valid European electronic ID (‘Europees erkend inlogmiddel’) to do this.
What will you need?
You will need the following information when completing the form:
- your citizen service number
- your bank account number
- your authorised person’s bank account number
- your authorised person’s personal information
Once you have completed the form
It is important that you and your authorised person both sign the completed form. Once this has been done, you can upload the form via Mijn UWV or send it by post to the address specified on the form.
