The employer or parent needs to apply for adoption or foster care pay between 4 and 2 weeks before the leave starts.
The employer can arrange adoption or foster care pay for their employee by using the Report an absence feature ('Verzuimmelder') in the employer portal ('werkgeversportaal') or through Digipoort, selecting 'WAZO-verlof'.
If the employer is unable to apply through the employer portal, they can complete the paper form Apply for WAZO pay (in Dutch, ‘Aanvragen WAZO-uitkering’).
The employer can let us know in the application who UWV should pay the adoption or foster care pay to.
Applying with a personal budget ('persoonsgebonden budget, pgb')
Budget holders ('budgethouders') need to apply for adoption or foster care pay on behalf of their care provider ('zorgverlener') using the form below. They can also ask the SVB ('Sociale Verzekeringsbank') to do this.
Care providers working through a personal budget ('pgb-zorgverlener') can also apply for adoption or foster care pay themselves by calling us.
Sending proof of adoption or foster care
Proof of adoption or foster care must always be sent after the application using the Submission form for additional documents for adoption or foster care pay (in Dutch, 'Nasturen documenten na aanvraag WAZO-uitkering'). The employer needs to ask the employee for this proof.
Parents who qualify for adoption or foster care pay, who do not have an employer, must apply for the pay themselves.
Parents who are receiving an unemployment benefit ('WW-uitkering') can apply for adoption or foster care pay through Mijn UWV. A DigiD account or a valid European electronic ID (‘Europees erkend inlogmiddel’) is needed to do this.
Parents in the following situations need to call us for the application form:
- parents who are receiving a sickness benefit ('Ziektewet-uitkering')
- parents who are receiving a salary-linked WGA benefit ('loongerelateerde WGA-uitkering')
- parents who are self-employed and have voluntary income protection coverage ('vrijwillige verzekering')
Proof of adoption or foster care must always be provided with the application.
We will make a decision within 4 weeks of receiving the complete application. Employees will receive the decision through Mijn UWV. Employers will receive a copy through the employer portal ('werkgeversportaal').
Parents have the right to object to our decision ('bezwaar maken').
