We will send you the life certificate form (‘verklaring van in leven zijn’) once a year. Make sure you complete this form within 6 weeks of receiving it. You can either complete the form online, or return it to us by post. 

If you want to Complete the life certificate form online you will need to log in to Mijn UWV using either your DigiD account or a valid European electronic ID (‘Europees erkend inlogmiddel’). You can complete the form as soon as you receive our letter.  

If you want to return the paper form by post, you must submit a copy of a current proof of identity along with your form. Make sure you write your citizen service number (‘BSN’) and the text ‘Kopie voor UWV’ on this document. Your photo and personal information must be clearly visible.  

If you want to return the paper form by post but have lost our letter, then you can download the life certificate form. 

If you are the next of kin and have received our letter, please call us to report the death. Or, you can complete and return the form provided. Doing so will ensure that the benefit is stopped in time, meaning that you will not have to pay anything back. You might also qualify for a bereavement support payment (‘overlijdensuitkering’).
