To cancel extended sick pay, you or your employee will need to complete the Cancellation form for extended sick pay (‘Beëindiging verlengde loondoorbetaling WIA’).

You will need the following when completing this form:

• your employee’s citizen service number
• your employee’s personal information
• your payroll tax number

If your employee’s contract or extended sick pay period is set to end in the next 13 weeks, you will not need to use the cancellation form. The extended sick pay period will automatically end if you do not renew it.

We will inform your employee about the WIA benefit long before their sick pay ends.

The extended sick pay period cannot be cancelled prematurely without our approval. If we approve the cancellation, the extended sick pay will end 13 weeks after we received your cancellation form.

You will receive a decision about your cancellation request within 2 weeks of us receiving your form.
