We need the medical information from your previous assessment to help an UWV physician (‘verzekeringarts’) conduct your disability employment assessment. We only use this medical information to assess your application.

We need this information because it will help us carry out the new evaluation more effectively. In some cases, reusing this information could also ensure we do not have to conduct the same assessment again. 

To give us permission, complete the Consent form for accessing medical information again for a disability employment assessment (‘Machtiging opnieuw gebruiken van medische gegevens voor aanvraag beoordeling werken met een voorziening’). You will need either a DigiD account or a valid European electronic ID (‘Europees erkend inlogmiddel’) to do this.

Submitting your completed form means you have given us permission to access the medical information we have on file for you. We recommend saving a copy of the form for your own records.
