You can only view and fill in the questionnaire if you have received a letter from us asking you to do so. It is important that you complete the questionnaire before the date mentioned in the letter.

You will need a DigiD account to access the medical questionnaire (in Dutch). It will take around 60 minutes to fill in.

If you are living abroad, you will need to fill in the medical questionnaire for when you are living abroad.

Please note: Make sure you have enough time to complete the questionnaire in 1 go. If you take a break for more than 15 minutes, you will be automatically logged out and will lose any information you have entered.

Make sure you have the following close to hand when completing the questionnaire:

  • information about your illness
  • details about your doctors and specialists 
  • information about your treatment 
  • information about your medication

Please note: If you have received documents from your doctor or specialist after completing the questionnaire, upload the information as soon as possible by using the form Submitting information for the assessment of your work capacity (‘Beoordeling van uw mogelijkheden’). This information will help us to complete our assessment more quickly. You should only use this form to upload documents.

You can check the status of your application via Mijn UWV from the 2nd day of reporting your illness.

We will send you a letter with a decision on your sickness benefit. If you are entitled to a sickness benefit, the letter will also state the amount you can expect to receive.

If you are living abroad, this form is also available in Dutch, French, German and Polish:
