If you want to apply for UWV support because you or your child have a disability, complete the disability support application form (‘Aanvragen voorziening’). You will need a DigiD account to do this.

You may be required to provide extra information while completing the form. You can do this by uploading this extra information as an attachment.

Please note: You will not be able to continue to the next step in the form if you do not attach this extra information. If you do not have this information to hand, you will be given 30 days to collect all the information you need.

Make sure you have UWV’s approval before purchasing any goods/services for your disability. This is important as UWV has contracts with specific suppliers and must purchase goods/services from these suppliers. If you make a purchase from a different supplier, UWV will not be able to reimburse you for this.

UWV will review your medical records to see whether you qualify for the type of support you have applied for. If you have applied for support in the past, we will have your medical information on file. If this is not the case, please send us this information within 14 days of sending us your application form.
