If you want to apply for UWV support because you or your child has a disability, complete the Disability support application form (‘Aanvragen voorziening’). You will need either a DigiD account or a valid European electronic ID (‘Europees erkend inlogmiddel’) to do this.
We might ask you to provide extra information after you have applied for disability support so that we can assess your application. Select the relevant form based on whether the information is medical or non-medical.
Submitting non-medical information
To submit general documents to us online, complete the Submitting non-medical information form (‘Nasturen algemene informatie’). You will need to log in to Mijn UWV using your DigiD account or a valid European electronic ID to do this.
If you prefer to submit the documents to us by post, download and complete the Submitting non-medical additional information form (‘Nasturen aanvullende informatie bij aanvraag voorziening’). Post the form and documents to us at the address given on the form.
Submitting medical information
If you have applied for disability support for you or your child and we do not have your medical report, you will need to submit it to us so that we can assess your application.
The medical report is a statement issued by a doctor or medical specialist, or an audiogram.

Make sure you have our approval before purchasing any goods or services for your disability. This is important as UWV has contracts with specific suppliers and must purchase goods/services from these suppliers. If you make a purchase from a different supplier, we will not be able to reimburse you for this.
