Reporting your work or benefit-related income

If you are entitled to an occupational disability benefit you will be asked to submit details of your gross income (‘bruto-inkomsten’) for a given period. We need this information to determine the amount of your benefit. You only need to fill in this form if you have received a letter from us asking you to do so.

How to complete the form

To access the form you will be asked to log in to Mijn UWV using your DigiD account, or a valid European login method. You will also need the following details:

  • your citizen service number (‘BSN’)
  • your work payslip information (‘loongegevens’)
  • your benefit payslip information

If you are able to provide all your payslips (‘loonstroken’) or proof of income for the period concerned, you do not need to complete the form. Instead, simply use the form to Upload your documents.

Decision
