Requesting return-to-work support

As an employer, we expect you to do everything you can to support your employee’s return to work. Sometimes this process can be costly, especially if your employee has a temporary contract or if the return-to-work process (‘re-integratie’) continues after their contract ends. In these cases, you might be entitled to financial support from UWV.
  • If the return-to-work process finishes before your employee’s contract ends, and the costs are higher than 70% of their gross wage (‘brutoloon’), we can cover the extra costs above 70% up to a maximum of € 3,000.
  • If the return-to-work process continues after your employee’s contract ends, and improves their chance of finding work, we can contribute up to € 3,000.
  • If the return-to-work process continues after your employee’s contract ends, and you use a return-to-work service provider (‘re-integratiebedrijf’), and your employee starts working for another employer for at least 6 months, we can reimburse costs up to € 5,000. We will pay the amount directly to the return-to-work service provider. 

Please note: If you are self-insured (‘eigenrisicodrager’) you are personally responsible for your employee’s return-to-work process. This means you will not be able to request return-to-work support.

  • Your employee’s return-to-work process must start at least 6 weeks before their contract ends.
  • You must submit your request no later than 4 weeks before the return-to-work process begins.
  • You and your employee agree on the Action plan (‘Plan van aanpak’).

Please note: You are responsible for supporting your employee during their absence by managing their sick leave and helping them return to work ('verzuimbegeleiding'). You are expected to work on this process together. Once your employee’s contract ends, we will take over responsibility for the return-to-work process.

To request return-to-work support, (‘Verzoek participatie in re-integratie’). You will need to log in to the employer portal with your EH3 level eHerkenning account to do this.

  • a copy of the Evaluation (‘Probleemanalyse’), the Action plan (‘Plan van aanpak’) and, if applicable, any changes to the Action plan or Evaluation (‘Bijstellingen’)
  • a proposal for an initial return-to-work plan along with an estimate of the costs
  • a recent payslip

Please note: You can also use this form only to upload your documents. Make sure you submit all the information as soon as possible as this will speed up the assessment of your application. 

As soon as we have received your application, you will receive a confirmation. If you want us to be involved in your employee’s return-to-work process, we will first need to check your return-to-work plan.

If you plan to hire a return-to-work service provider, we will need to check whether this provider meets our quality criteria.
