To inform us of your employee’s recovery, use the ‘Report an absence’ feature on the employer portal. You will need an EH3 level eHerkenning account to do this. Alternatively, if you are a Digipoort user, you can inform us of your employee’s recovery through your own HR administration system, which is directly linked to our system.

If you cannot upload the form online

If you do not have access to the employer portal or Digipoort, download and complete the Report an employee’s recovery form (‘Mijn werknemer in de Ziektewet is weer (gedeeltelijk) aan het werk of weer volledig ziek’).

If you are self-insured (‘eigenrisicodrager’) and you do not have access to the employer portal or Digipoort, download and complete the Report an employee’s recovery as a self-insured employer form (‘Hersteldmelding (ex-)werknemer voor werkgevers die eigenrisicodrager zijn voor de Ziektewet’). Post the completed form to us at the address given on the form.
