The person you appoint must be a doctor, lawyer or other legal professional. Select the relevant form and report the authorised person’s name via the employer portal (‘UWV werkgeversportaal’). You will need to log on with your eHerkenning account in order to access the form:
- Reporting an authorised person for medical information on a sickness benefit (‘Medisch gemachtigde voor Ziektewet-beslissing doorgeven’)
- Reporting an authorised person for medical information on a WIA benefit (‘Medisch gemachtigde voor WIA-beslissing doorgeven’)
- Reporting an authorised person for medical information on an unemployment benefit in the public sector and education (‘Medisch gemachtigde voor WW-beslissing doorgeven’)
Please note: Medical information is strictly confidential (‘medisch geheim’) and we are required by law to protect the privacy of the employee. We do not send any medical information to you directly, and the authorised person is not allowed to share the medical information with you.
To access the medical authorisation forms, you will need to arrange authorisation through your eHerkenning provider for these services:
- Self-insured for the sickness benefit (‘Eigenrisicodrager Ziektewet’).
- Self-insured for the WIA benefit.
- Self-insured for the unemployment benefit public sector and education (‘eigenrisicodrager WW Overheid en Onderwijs’).
- Blank form.
Once you have done so, you will see a new menu item in the employer portal called ‘Notify medical representative’ (‘Medisch gemachtigde doorgeven’).
We will check if you have a direct interest in the decision and that the authorised person meets the conditions. If so, we will send the medical information to the person you have chosen. Otherwise, we will send you a letter explaining why we cannot share the information.
